Photographer’s Release Form
Photos taken by a professional photographer require a Photo Release Form. If you do not provide us with the form, you are then confirming that you or someone other than a photographer took the pictures that you are including in your photo card.
If you have a studio image, for example: school portraits, sports photos, cruise portraits, etc. that does not indicate who created the pictures/portraits/work and you are not aware who created the work you may be able to claim that the work you are requesting is for "fair use," and that you are unaware, or are unable to contact the photographer.
Copyright Indemnification: Purchaser agrees to hold harmless Photo Card Creations by Terri for any and all costs, expenses that may or may have occurred, attorney's fees and/or judgments resulting from claims or lawsuit made against Photo Card Creations by Terri for any reasons claiming that the work produced violates any copyright or proprietary right of any person or entity.
Signature of Photographer's Name ___________________________________ (Please Print) grants permission to Photo Card Creations by Terri to reproduce the copyrighted images taken on _____________ (date) for____________________________________ (Your Name).
The images will be reproduced on a photo card. Signature of the photographer releases Photo Card Creations by Terri from any copyright infringement.
